Position Description :

The Nurse Practitioner is a member of an interdisciplinary care team providing episodic care, wound care, basic needs, and service connection referrals. The clinical team operates as a mobile unit working across various areas in the city, serving individuals who face barriers in accessing care and services. The team has quick access to in-house Social Workers, Outreach Workers, Community Developers, and support staff specializing in Mental Health, Systems Navigation, Diabetes, and Chronic Diseases, all while out in the community.
