The Nurse Practitioner (NP) reports to the Executive Director and collaborates with the resident, family / caregiver, and health care team in the development, implementation and evaluation of the residents’ plan of care, as well as provides leadership and mentorship to team members that enhances their knowledge, assessment skills, and ability to care for residents.

Responsibilities :

  • Diagnose residents, order and interpret diagnostic tests, and prescribe medication and other treatment,
  • Provide direction and leadership to RN's, RPN’s, PSW’s, and other direct care team members in all aspects of resident care, within established legislative standards,
  • Complete nursing documentation, and assist residents with move-in,
  • Is inspirational and passionate about quality dementia care and capturing positive moments
  • Work collaboratively with attending physicians,
  • Contribute positively to a team approach in being person centered with one another,
  • Ensure medication reconciliation occurs as needed,
  • Implement resident-focused programs, policies, and procedures with the Director of Nursing and the Quality Team,
  • Provide education to clinical team and offer in-services,
  • Liaise with Residents, families and outside agencies as required Respects the Residents' Bill of Rights
  • Any and all other tasks as assigned

Qualifications :

The qualifications needed to join our family are as follows :

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Post-graduate education and experience in gerontology (recommended)
  • Experience working as a Nurse Practitioner,
  • Experience in long-term care, with providing empathetic care for the elderly,
  • Experience providing leadership and supervision in any healthcare environment.
  • Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals
  • Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory reference
