The Nurse Practitioner (NP) reports to the Executive Director and collaborates with residents, families/caregivers, and the healthcare team in developing, implementing, and evaluating residents’ care plans. They provide leadership and mentorship to team members, enhancing their knowledge, assessment skills, and ability to care for residents.

Responsibilities :

  • Diagnose residents, order and interpret diagnostic tests, and prescribe medication and other treatments.
  • Provide direction and leadership to RNs, RPNs, PSWs, and other care team members within legislative standards.
  • Complete nursing documentation and assist residents with move-in processes.
  • Be passionate about quality dementia care and capturing positive moments.
  • Collaborate with attending physicians.
  • Promote a person-centered team approach.
  • Ensure medication reconciliation occurs as needed.
  • Implement resident-focused programs, policies, and procedures with the Director of Nursing and Quality Team.
  • Provide education and in-services to the clinical team.
  • Liaise with residents, families, and outside agencies as required, respecting the Residents' Bill of Rights.
  • Perform any other tasks as assigned.

Qualifications :

The qualifications needed to join our team include:

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario or a comparable regulatory authority in Canada, with independent practice rights in Ontario under Bill 60.
  • Post-graduate education and experience in gerontology (recommended).
  • Specialty certification in gerontological nursing (recommended).
  • Experience working as a Nurse Practitioner.
  • Experience in long-term care and providing empathetic elderly care.
  • Leadership and supervision experience in healthcare settings.
  • Ability to utilize various team skills to maximize well-being.
  • Must have a recent vulnerable sector screen (within 6 months), TB test results (within 6 months), and three supervisory references.
