  • Promote safe resident care by mitigating harm.
  • Liaise with discharge planning services.
  • Collaborate to improve resident care continuity.
  • Dedicate time to both direct resident care and education.
  • Exercise independent judgment and provide supervision.
  • Participate in after-hours and on-call coverage.

NURSE PRACTITIONER (NP) (Full-Time – Independent Contractor) – Extendicare Rouge Valley

Extendicare Rouge Valley seeks a self-directed, results-oriented nursing leader with strong communication and critical thinking skills. You will provide innovative clinical leadership for our inter-professional team, focusing on geriatric care, and collaborate with physicians, staff, and system partners.

The Nurse Practitioner is a key member of the leadership team, working within a strong interdisciplinary team, reporting to the leadership team, and providing direct comprehensive care to residents with complex needs, based on advanced clinical knowledge and skills. The practice is interdependent, working collaboratively within the team, with a scope beyond regular nursing care as defined by the College of Nurses of Ontario.

Responsibilities
  • Deliver primary health care to residents, managing acute and chronic conditions, health promotion, and disease prevention.
  • Provide clinical leadership for the Behavioral Support Transitional Unit and Long-term care team, including resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and responsive behavior management.
  • Conduct health assessments, perform physical examinations, interpret findings, order and interpret diagnostic investigations.
  • Develop person-centered care plans in collaboration with residents and families, using clinical reasoning to diagnose and assess health risks.
  • Communicate assessment findings, diagnoses, and treatment plans effectively.
  • Select appropriate interventions to restore or maintain resident health and functionality.
  • Promote safe resident care by mitigating harm and addressing risks.
  • Liaise with discharge planning services for hospitalized residents.
  • Collaborate on initiatives to improve resident care continuity and participate in educational, quality improvement, and research activities.
  • Dedicate 80% of time to direct resident care and 20% to research and education.
  • Incorporate knowledge of diversity, cultural safety, and health determinants in practice.
  • Exercise independent judgment, work effectively within a team, and provide supervision and mentorship.
  • Build partnerships with stakeholders and communicate effectively.
  • Participate in after-hours and on-call coverage as needed.
  • Maintain knowledge of RNAO guidelines and relevant regulations.
