The Nurse Practitioner reports to the Director of Care and collaborates with the Medical Director, residents, families, and healthcare team to develop, implement, and evaluate care plans. The role includes providing leadership and mentorship to staff, leading research, and engaging in evidence-based practices to improve outcomes.

Responsibilities
  1. Provide primary care in collaboration with physicians and the healthcare team.
  2. Consult and collaborate on residents’ care plans.
  3. Support and guide staff in clinical situations.
  4. Make referrals to specialists and other providers.
  5. Conduct health assessments, including medication reviews.
  6. Participate in care conferences and research initiatives.
  7. Perform screening and diagnostic investigations, interpret results, and follow up.
  8. Identify and implement care innovations based on research.
  9. Perform other duties as assigned.
Qualifications
  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario or equivalent registration in another Canadian jurisdiction.
  • Knowledge of evidence-based clinical practices and long-term care concepts.
  • Excellent communication and interpersonal skills.
  • Post-graduate education in gerontology (preferred).
  • Experience as a Nurse Practitioner and in long-term care (preferred).
  • Leadership experience in healthcare.
  • Ability to work collaboratively and focus on patient well-being.
  • Willing to provide a recent Vulnerable Sector Check and meet health requirements.
  • Provide references and proof of required vaccines.
