Your role as a Nurse Practitioner

The mission of the Nurse Practitioner’s role at Dialogueis to accompany the patients in their journey with Dialogue by offering front-line medical support and symptom evaluation. Collaboration and learning are both significant aspects of this role. What you’ll be doing:

• Conducting remote assessments via the Dialogue Care Platform where you will evaluate symptoms, diagnose and provide treatment for both common and complex conditions
• Delivering high-quality patient care by offering timely medical advice and identifying urgent symptoms
• Creating personalized care plans tailored to your patient's needs
• Collaborating closely with an interdisciplinary team of doctors, nurses, care coordinators and care managers, psychologists, etc to provide comprehensive, high-quality care
• Educating and supporting patients on preventative care, health management and lifestyle changes
• Referring patients, when needed, to specialists or allied healthcare professionals and community services
• Maintaining detailed records of patient interactions, treatment plans, and follow-ups that adhere to regulatory standards
• Contributing to internal projects or initiatives
