Job summary

Windsor Essex Community Health Centre is seeking a Nurse Practitioner at the Leamington site to backfill maternity leave, conducting assessments and delivering care in collaboration with an inter-professional team. The role emphasizes patient-centred care, health education, and outreach to improve access to primary health services.

The position requires an Extended Class NP with community health experience, strong communication skills, and the ability to work across diverse populations;

Qualifications

• Undergraduate and/or Graduate degree in Nursing from a recognized University.
• Qualified to provide care to clients across their lifespan.
• Member in good standing in the Extended Class with the College of Nurses of Ontario.
• Member in good standing with the Nurse Practitioner’s Association of Ontario (NPAO) and/or RNAO.
• Training and/or certification related to mental health and addictions recommended considered an asset.
• Experience with phlebotomy.
• Completion of a Police Clearance and Vulnerable Sector Check prior to commencing employment.

Responsibilities

• Perform assessment, diagnosis and treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Extended Class NPs as well as the weCHC Medical Directives.
• Participate in active outreach to assist client access to primary health care and connect to community physicians/inpatient services.
• Work collaboratively with consulting physician and health care team to provide optimal care.
• Provide clinic assessments addressing physical and emotional needs of clients with multiple co-morbid conditions.
• Contribute to chart documentation, chart reviews and case conferences.
