About Vancouver Acute Home Based Treatment (AHBT)

AHBT provides short-term, in-home mental health and substance use care for clients aged 17 and up, offering an alternative to hospitalization. The team operates 7 days a week, providing intensive outreach post-discharge from acute care in Vancouver. The NP will work within a team-based care model alongside RNs, care coordinators, psychiatrists, and other healthcare providers, focusing on mental health, addiction medicine, and primary care.

Responsibilities of a Nurse Practitioner at VCH
  • Conduct comprehensive assessments, diagnose diseases, and manage treatment plans.
  • Initiate treatment, including prescribing medications and therapeutic interventions, following statutory and employer standards.
  • Provide guidance to healthcare professionals and participate as an autonomous team member.
  • Collaborate with clients and professionals to assess health trends and patterns.
  • Develop population health strategies and participate in policy and quality improvement activities.
  • Engage in peer and self-review to evaluate service outcomes.
  • Maintain professional development and clinical privileges.
  • Report to an Operational Director, ensuring quality assurance and patient safety.
Qualifications
  • Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
  • Experience as an NP is preferred but motivated newcomers are encouraged to apply.
Knowledge & Skills
  • Ability to perform full NP scope of practice.
  • Knowledge of developmental stages, pathophysiology, psychopathology, epidemiology, pharmacology, and evidence-based practice.
  • Excellent communication and collaboration skills.
  • Ability to evaluate research literature and implement evidence-based practices.
  • Leadership skills within team settings.
  • Physical ability to perform job duties.
