The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 120 physicians across 19 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive collaborative primary health care to over 158,000 patients.

Family Health Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care.

Duties and Responsibilities:
  1. Assess holistically and provide services to patients in all developmental stages
  2. Complete health assessments, including health history and physical examination
  3. Formulate and communicate medical diagnoses
  4. Determine the need for, and order from, an approved list of screening and diagnostic laboratory tests and interpret the results
  5. Monitor the ongoing therapy of patients with chronic stable illness by providing effective pharmacological, complementary or counseling interventions
  6. Prescribe medication according to scope of practice
  7. Provide health education and health promotion information to individuals and groups
  8. Consult with physician in accordance with the standards for consultation with physicians, and/or refer the client to another health care professional
  9. Solid understanding and experience with adult group learning principles, teaching self management techniques & ability to evoke participation in a group environment
  10. Level of comfort or willingness to become familiar with virtual meeting technology
  11. Experience with facilitation of an online program considered an asset
Qualifications:
  1. Baccalaureate Degree in Nursing
  2. Primary Health Care Nurse Practitioner Certificate
  3. Current Registration with the College of Nurses of Ontario in the Extended Class
  4. Current membership in RNAO or NPAO
  5. 2 years experience in primary care
  6. Demonstrated ability to work effectively as a member of an interprofessional team, with excellent interpersonal and communication skills
  7. Must have access to reliable transportation as travel between sites is required

Hourly rate: $54.4598 - $66.4144 (based on experience)
