We are currently recruiting for Nurse Practitioner to provide services to MD Care Family Health Organization. While these positions are employed by Community Healthcaring Kitchener-Waterloo, the Nurse Practitioner’s day-to-day work location (across 3 locations), schedule of hours, as well as applicable policies and procedures, will be determined by MD Care Family Health Organization Team to which they are assigned.

Job Summary
The Nurse Practitioner provides care within the MD Care Family Health Organization interdisciplinary team, working collaboratively with physicians and other health care professionals to support patients and families across the lifespan. The Nurse Practitioner helps improve access to care, enhance health outcomes, and support coordinated, team-based primary care for diverse patient populations
