COMPENSATION: Starting from $141,494 – $160,312 plus comprehensive health benefits. Dedicated time for non-clinical work. Relocation benefits available.

LOCATION: This position is required to work onsite at 625 Powell Street, Vancouver, BC Centre for Excellence in HIV/AIDS.

ORGANIZATION:

The BC Centre for Excellence in HIV/AIDS (BC-CfE) is Canada’s largest HIV/AIDS research, treatment and education facility, recognized internationally as a leader in combating HIV/AIDS and related diseases. A multidisciplinary team of clinicians, laboratory scientists, health economists, epidemiologists, clinical researchers, statisticians, programmers, and data analysts work collaboratively to improve the health of British Columbians living with HIV and other chronic conditions.

The BC-CfE opened the Hope to Health Research and Innovation Centre (H2H) in late 2019, providing interdisciplinary primary care services in Vancouver’s Downtown Eastside. H2H’s team-based approach to primary health care is designed to engage marginalized populations effectively.

PROGRAM:

The Downtown East Side Hope to Health Integrated Complex Primary Care (H2H) clinic offers an innovative, low-threshold, client-centered, proactive, highly coordinated, comprehensive complex primary care model.

The clinic aims to serve clients with complex medical or social needs in the Downtown Eastside, focusing on HIV, Hep C, mental illness, substance use, and other chronic illnesses.

The H2H Centre includes harm reduction programs and selected specialty services tailored to clients' medical profiles. Medical and psychosocial/behavioral interventions are delivered by an integrated interdisciplinary team, supported by ongoing research, monitoring, evaluation, and quality improvement initiatives.

ROLE:

We are recruiting Nurse Practitioners to join a supportive multidisciplinary care team that includes an RN, LPN, social worker, peer support worker, MOAs, and access to other health providers such as family physicians, PT, OT, dietitian, psychologist, clinical pharmacist, phlebotomist, and psychiatry consulting services. The position is part-time (0.3-0.5 FTE), with a preference for full-time. The patient population primarily consists of complex clients living in the DTES, many with co-morbidities including mental health, substance use disorders, addiction, and psychosocial issues. The clinic promotes learning, skill development, and continuous quality improvement through clinical simulations and various initiatives.

RESPONSIBILITIES:

  • Diagnose and treat undiagnosed clients within the Nurse Practitioner scope, including writing orders for treatment and medications, and providing emergency first-line care.
  • Monitor ongoing care, order and interpret diagnostic investigations, and plan treatment accordingly.
  • Prioritize health management, provide follow-up, communicate diagnoses and treatment plans with clients and families, and support clients’ responses to health conditions.
  • Collaborate with physicians and other health/social service providers to assess and develop treatment plans.
  • Prescribe medications within statutory and employer policies.
  • Participate in research to improve client care and contribute to nursing and health policy advancements.
  • Implement screening and health promotion activities for at-risk populations.
  • Other duties as required.

Please note, the H2H clinic does not provide prenatal, postnatal, or pediatric services (13 and under, or in exceptional circumstances).

SCHEDULE:

  • 8-hour shifts
  • Monday to Friday

QUALIFICATIONS, EDUCATION, TRAINING, AND EXPERIENCE:

  • Current registration as a Nurse Practitioner with the BC College of Nurses and Midwives (BCCNM) plus 4 years of relevant clinical experience.
  • Successful completion of the Objective Structured Clinical Examination (OSCE).
  • Basic Cardiac Life Support (Level C).
  • Ideally, 2+ years of work experience as an NP.

SKILLS & ABILITIES:

  • Relevant clinical experience or interest in substance use care, with ability to support clients with addiction medicine needs.
  • Knowledge of trauma-informed and culturally safe care practices.
  • Preference for candidates with expertise in inner-city primary care and substance use disorders.
  • Ability to work collaboratively within the primary care team.
  • Good standing with the BC College of Nurses and Midwives (BCCNM).
