Job summary

Superior Family Health Team in Sault Ste. Marie is seeking a Nurse Practitioner to join our collaborative clinic. You will manage your own patient roster, delivering comprehensive primary care including acute and episodic treatment, health promotion, and disease management.

You will collect PHIPA-compliant data, coordinate with a multi-disciplinary team, and provide consultation as part of high-quality patient care. Flexible hours and team support are available.

Qualifications

  • Current registration with the College of Nurses in Ontario in the Extended Class in good standing.
  • The ability to work in a rapidly changing environment.
  • Demonstrated ability to organize, prioritize and multitask efficiently.

Full Time Permanent 37.5 Hour Per week

Superior Family Health Team is looking for a motivated Nurse Practitioner to join our collaborative, multi-disciplinary clinic. In this role, you’ll work alongside a team of allied health professionals and community partners to provide high-quality, patient-focused care. You will manage your own patient roster, delivering comprehensive primary care that includes acute and episodic treatment, health promotion, disease prevention, and chronic disease management. Collect and maintain accurate data and information in accordance with the Personal Health Information Act (PHIPA) and the requirements of any regulatory body standards. Effective listening, observation, and facilitation skills to effectively assess the patient and provide appropriate treatment and consultation. Other duties may be assigned
