- What We’re Looking For:
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice with no restrictions and in good standing.
- Nurse Practitioner Certification from a recognized program
- Membership with Nurse Practitioners’ Association of Ontario (NPAO)
- National certification in Gerontology is an asset
- Minimum 1 year experience in Long-Term Care and/or Retirement Care sector
- Provide and maintain current CPR Certificate for the duration of employment
- Knowledgeable regarding cultural competency in professional nursing practice
- Excellent communication and people skills
- Working knowledge of Point Click Care is an asset
- Knowledge of RAI/MDS and BSO is an asset
- Tuberculosis testing (Two-Step Mantoux)
- Provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
- Proof of immunization according to Plan A Policy
- Key Responsibilities:
- Remain accountable to the Nurse Practitioner practice standard in addition to the other practice standards and guidelines all nurses must follow
- Accountable to the Medical Director and Director of Care of the Home for meeting the Long-Term Care or Retirement Home’s policies and procedures and protocols for all medical services
- Adhere to the regulatory bodies such as the Fixing Long-Term Care Act, 2021 (or Retirement Homes Act, 2010)
- Maintain confidentiality and act in an ethical manner consistent with professional expectations and the home’s code of conduct
- Collaborate with the resident, family, and health team in the development, implementation, and evaluation of the residents’ plan of care
- Lead and collaborate in research, education, and evidence-based practice initiatives to optimize resident outcomes
- Lead and/or collaborate on all complex, high-risk clinical situations as a contributing subject matter expert
- Diagnose, order, interpret diagnostic tests, and prescribe medication and other treatment
- Provide consistent and effective assessments, therapeutic interventions, follow-up care, and monitoring, ensuring all documentation is provided according to prescribed timelines
- Encourage and model critical thinking that identifies needs and makes recommendations for changes within the home to enhance best practice outcomes
- Participate in any quality improvement projects (i.e., Antipsychotic Reduction, ED transfer reductions, Pressure Ulcers, Fall Prevention)
- Provide education and coaching
- Contribute to the Palliative Care Team goals
- Develop, implement, and evaluate policies and procedures related to nursing, interdisciplinary care, and clinical practices as assigned
- Participate in creating an organizational environment that supports the safety and quality of resident care and life, collaborative practice, and professional growth
