Job summary

Southbridge Care Homes is seeking an Attending Nurse Practitioner for full-time permanent placement in Thunder Bay, Ontario. The Nurse Practitioner will be responsible for providing comprehensive primary care to residents, collaborating with an interdisciplinary team, and enhancing the care quality through leadership and mentorship. Ideal candidates will have a strong clinical background and leadership capabilities, and will meet the legislative scope of practice outlined by the College of Nurses of Ontario.

Qualifications

  • Registered Nurse designation in Ontario.
  • Must have a Nurse Practitioner certificate.
  • Experience in primary care or long-term care settings preferred.

Responsibilities

  • Develop, implement, and evaluate care plans in collaboration with residents and health teams.
  • Provide leadership and mentorship to LTCH staff.
  • Conduct health assessments and medication reviews.
  • Support nursing staff in clinical situations.

Nurse Practitioner

ATTENDING NURSE PRACTITIONER

The Nurse Practitioner practice is an interdependent practice in which the Nurse Practitioner works collaboratively within an interdisciplinary team. The scope of the NP’s practice extends beyond regular nursing care.

Responsibilities

  • The Attending Nurse Practitioner (NP) reports to the Director of Care and is accountable to the Medical Director for meeting the Long Term Care Home’s (LTCH) policies, procedures, and protocols for medical services. The NP is a primary care provider to residents and works within her/his legislative scope of practice as outlined by the College of Nurses of Ontario.
  • The NP collaborates with residents, families/caregivers, and the health team to develop, implement, and evaluate residents’ care plans. The NP provides leadership and mentorship to LTCH staff, enhancing their assessment skills and ability to care for residents. The NP also leads research, education, and evidence-based practice initiatives to improve outcomes for residents, LTCH, and health systems.
  • The NP maintains confidentiality and acts ethically, in accordance with professional standards, the written agreement with the LTCH, and the facility’s policies and code of conduct.
  • To ensure continuity of care, the NP collaborates, consults, and refers as appropriate:
  • Works with physicians, nursing staff, interdisciplinary team members, and external resources regarding care plans.
  • Supports and guides nursing staff and team members in clinical situations.
  • Makes referrals to specialists and other health providers.
  • Accepts referrals from team members.
  • Plays a key role in clinical programs, supporting staff and physicians for efficient practice.
  • Provides comprehensive primary care as part of the team.
  • Performs health assessments on admission, annually, and as needed, including medication reviews.
  • Conducts health histories and collaborates with residents and families to develop care plans.
  • Engages residents in discussions about their care and health priorities.
  • Provides health education tailored to residents’ needs.
  • Participates in care conferences.
  • Manages residents' care with pharmacological, complementary, and counseling interventions, and performs procedures within scope.
