Responsibilities

  • Coordinate patient care and ensure seamless care delivery.
  • Conduct patient intake, triage, and initial assessments.
  • Diagnose illnesses and develop appropriate treatment plans.
  • Perform comprehensive Total Health Assessments and periodic health reviews.
  • Order and interpret laboratory tests.
  • Provide counseling on mental health concerns.
  • Educate patients on preventive healthcare.
  • Prescribe medications according to clinical guidelines.
  • Develop and implement follow-up care plans.
  • Facilitate referrals and coordinate diagnostic testing.
  • Work closely with dietitians and kinesiologists.

TheNurse Practitionerwill assess and manage patients with an emphasis on prevention and optimizing health. Working collaboratively with a multidisciplinary team—including physicians, dietitians, kinesiologists, and an athletic therapist—they will develop and implement personalized health plans, including referrals, counselling, and treatment.

Duties and Responsibilities:

The key job functions are:

  • Coordinate patient care and ensure seamless care delivery.
  • Conduct patient intake, triage, and initial assessments.
  • Diagnose illnesses and develop appropriate treatment plans.
  • Perform comprehensive Total Health Assessments and periodic health reviews.
  • Order and interpret laboratory tests to support clinical decision-making.
  • Provide counseling on mental health concerns and lifestyle modifications.
  • Educate patients on preventive healthcare and wellness strategies.
  • Prescribe medications in accordance with clinical guidelines.
  • Develop and implement follow-up care plans based on patient assessments.
  • Facilitate referrals and coordinate diagnostic testing in collaboration with family physicians.
  • Work closely with dietitians and kinesiologists to support patient health goals.

Education:

  • Master of Nursing – Nurse Practitioner or post-master’s diploma required
  • Current CARNA registration in good standing
  • Valid CPR certification

Experience:

  • Previous experience as a Nurse Practitioner required
  • Strong knowledge and experience in primary care required
  • Experience in chronic disease management preferred
  • Advanced clinical nursing skills within the scope of a Family Nurse Practitioner (FNP); additional relevant training is an asset
  • Experience in mental health is an asset

Base Salary:$120,000 + per year

Work Hours:

  • 40 hours per week

Remote Work:

  • This position is not remote and required to be on-site.
