Job description

Harbinger Network is partnering with a well-established community-based healthcare team to find a Nurse Practitioner for their Community Outreach team. This role is ideal for newly licensed Nurse Practitioners eager to make a community impact. All training and full support from management and staff are provided.
Role Details
  1. Hosting medical clinics
  2. Collaborating with a team of professionals
  3. Conducting home visits as needed
  4. Maintaining strict privacy and confidentiality
  5. Assisting vulnerable community members
  6. Approaching situations with empathy
  7. Leading vaccination clinics
  8. Liaising with primary care resources
  9. Reporting to a manager with 7 years of community experience
Benefits
  1. Salary: $112,000 - $126,000 annually

Qualifications
  1. Recognized Primary Care Nurse Practitioner program completion
  2. Baccalaureate Nursing Degree with Extended Class registration (Ontario College of Nurses)
  3. Own vehicle, driver’s license, and insurance
  4. Experience in community health with all ages (preferred)
  5. Understanding of the Index of Well-Being and social isolation impacts
  6. Strong emotional intelligence, communication, conflict management, and problem-solving skills
  7. Ability to set and maintain boundaries
  8. Excellent English communication skills; second language is an asset
  9. Proficiency with computers and software (Microsoft Office, email, internet)
  10. Ability to obtain a Police Vulnerable Sector Check
