Harbinger Network is partnering with a well-established community-based healthcare team to find a Nurse Practitioner for their Community Outreach team. This role is ideal for newly licensed Nurse Practitioners eager to make a community impact. All training and full support from management and staff are provided.
Role Details
- Hosting medical clinics
- Collaborating with a team of professionals
- Conducting home visits as needed
- Maintaining strict privacy and confidentiality
- Assisting vulnerable community members
- Approaching situations with empathy
- Leading vaccination clinics
- Liaising with primary care resources
- Reporting to a manager with 7 years of community experience
Benefits
- Salary: $112,000 - $126,000 annually
- KM reimbursement ($500-$600/month)
- HOOPP pension from day one
- Three weeks' vacation initially, increasing to four in the second year

Qualifications
- Recognized Primary Care Nurse Practitioner program completion
- Baccalaureate Nursing Degree with Extended Class registration (Ontario College of Nurses)
- Own vehicle, driver’s license, and insurance
- Experience in community health with all ages (preferred)
- Understanding of the Index of Well-Being and social isolation impacts
- Strong emotional intelligence, communication, conflict management, and problem-solving skills
- Ability to set and maintain boundaries
- Excellent English communication skills; second language is an asset
- Proficiency with computers and software (Microsoft Office, email, internet)
- Ability to obtain a Police Vulnerable Sector Check
