The Elderhood Care Team is seeking a highly motivated and compassionate Nurse Practitioner to provide primary geriatric and palliative care services. This role combines mobile home care and clinic visits, focusing on chronic disease management and holistic patient well-being, playing a critical part in improving the lives of the elder community.
Job Summary The Elderhood Care Team and Focus Palliative Care Services, under the direction of Dr.
Di Cintio, are seeking a highly motivated and compassionate Nurse Practitioner to provide primary geriatric and palliative care services in both clinic and mobile home care settings.
This role offers the opportunity to work at the forefront of geriatric healthcare, focusing on preventative care, chronic disease management, and holistic patient well-being.
Additionally, patients 18+ requiring palliative care are also supported.
Who We Are A team-based, OHIP-covered primary and palliative care service based in Cornwall, Ontario, offering mobile house calls and in-clinic visits with a holistic approach for older adults with advanced care needs.

Providing care in accordance with the Standards of Practice for Registered Nurses in the Extended Class and Expanded Services for Patients Provide palliative care services to patients, focusing on comfort, symptom management, and quality of life Requirements Current and valid registration and certificate in the Extended Class with the College of Nurses of Ontario (without restriction) Certification as a Primary Health Care NP, in Ontario Baccalaureate Degree in Nursing from an approved University, Master of Science (Nursing) Current member of NPAO / CNO Exceptional understanding of current, evidence based methods and practices of primary health care delivery Strong understanding of all MAiD legislation and processes How to apply Please submit your resume and a cover letter outlining your interest and qualifications to [emailprotected] .
