Shift: M-F 8:00-4:30 pm with rotating weekend and holidays
The Nurse Navigator will be responsible for coordinating services and supporting identified patients through a complete health system. This includes assisting with access issues, identifying resources, providing educational materials, and developing relationships with service providers and the community. The Nurse Navigator coordinates care in a timely and efficient manner, identifying and eliminating barriers to care. They serve as a consistent care coordinator throughout the continuum of care, assessing the physical, psychological, and social needs of patients to enhance outcomes, increase satisfaction, and reduce costs. Additionally, the Nurse Navigator is responsible for community education, screening, monitoring outreach efforts, and promoting survivorship.

The estimated total compensation for this role ranges from $35.35 to $47.58, including wages, benefits, and other forms of compensation. The exact starting salary will be determined at the time of hire, considering factors such as skillset, experience, education, credentials, and licensure.
