Job Summary :
The Nurse Navigator is a health care professional with specialized knowledge of oncology who provides guidance, support, and self-management capacity to patients and families who are strongly suspected of having cancer or have a new diagnosis of cancer. The Nurse Navigator is assigned permanent responsibility for the nursing activities of Cancer Navigation Services.
The Nurse Navigator works independently to make clinical decisions and provides support the primary care provider by helping to verify the required referrals and tests have been ordered and providing guidance on the work-up of suspected cancer
Through case navigation and comprehensive assessment, the Nurse Navigator is able to assess, recognize and address a wide range of physical, psychosocial, emotional, and practical needs that may arise. The incumbent will move the patient through the system by creating a navigation plan that addresses patient and family’s needs and by facilitating access and connections to supportive services, appropriate referrals, surgery, primary care, and other specialist care such as Medical Oncology and Hematology.
The Nurse Navigator, in collaboration with cancer navigation services and key partners, participates in ongoing quality improvement toward enhancing nurse navigator evidence-based and person-centred care to advance equitable cancer outcomes.
