Job Overview:

The Nurse Liaison will work with the Director of Pre Admissions to develop the inpatient and long-term care census aligned with Children's Specialized Hospitals' organizational goals and strategic plan.

Qualifications:

Required:

  • Nursing Degree
  • Minimum 2 to 5 years of clinical experience with pediatrics, including rehabilitation
  • Ability to assess patients safely and accurately for pediatric rehabilitation admissions
  • Experience working with families, healthcare teams, agencies, and hospital departments
Scheduling Requirements:
  • Shift: 9:00 AM - 5:00 PM, Days to work
  • Full-Time
Essential Functions:
  • Review and document inpatient or long-term care referrals, guiding next steps with referring entities
  • Communicate promptly and effectively with stakeholders and families regarding the intake process and documentation
  • Electronically document case notes for transparent communication
  • Proactively contact hospitals and agencies to gather vital information and prevent delays
  • Ensure necessary information is obtained and shared appropriately
  • Local travel required
  • Perform other duties as assigned
