Job description
Overview

The Cardiac Department is seeking a highly organized, clinically experienced, and patient-centered Cardiac Triage Coordinator to join our dynamic and multidisciplinary Cardiac Department. This critical role supports the cross-coverage and delivery of seamless and timely care across four specialized cardiac programs: Cardiac Surgery, Structural Heart, Interventional Cardiology (Cath Lab), and Electrophysiology (EP).

The Cardiac Triage Coordinator is responsible for the intake, clinical review, and prioritization of referrals across these programs, ensuring that patients are navigated through the system with efficiency, safety, and compassion. By working closely with physicians, nurse practitioners, diagnostic teams, and administrative staff, the coordinator helps to streamline communication, reduce wait times, and facilitate timely access to assessments, procedures, and surgical interventions.

This role serves as a vital liaison between patients, referring physicians, and internal/external teams. It requires strong clinical judgment, excellent organizational skills, and a commitment to service excellence. The successful candidate will help ensure that patient flow is optimized, referrals are appropriately triaged, and care is delivered according to urgency and clinical need, in alignment with organizational standards and ministry guidelines.
