What the Program Entails
The Homelessness and Addiction Recovery Treatment (HART) Hub provides integrated support to people experiencing complex health and social issues related to substance use and addictions: and aims to connect people who are experiencing homelessness or are at risk of losing their housing and/or people who have mental health, addictions and substance use health needs to treatment and preventative services, while addressing basic needs such as food, clothing and showers.
What You’ll Do
The Nurse Care Coordinator plays a central role in supporting clinical operations and ensuring high-quality, coordinated care within the HART Hub—an interdisciplinary program serving individuals with complex health and social needs. This role involves working closely with the HART Hub team to promote integrated, client-centered care across multiple SWCHC sites and services.
In the initial phase, the Nurse Care Coordinator will focus on supporting clients entering the supportive housing program, collaborating closely with the Nurse Practitioner to help ensure successful transitions and sustained tenancies. This role will continue to evolve as the program develops.
Key responsibilities include clinical coordination, supervision of nursing staff, crisis intervention, and contributing to policy and program development. The Nurse Coordinator also plays a key role in quality improvement initiatives, supports compliance and reporting activities, and works collaboratively with the HART Hub leadership team to identify service gaps and guide program priorities.
The Nurse Care Coordinator will have an active outreach component, engaging with clients in local rooming houses within the Somerset catchment. They will work alongside the HART Hub team to identify and support individuals who may benefit from the supportive housing model. Additionally, the Nurse Coordinator will contribute to the development of medical directives that allow the nursing team to work autonomously within their scope.
