What the Program Entails

The Homelessness and Addiction Recovery Treatment (HART) Hub provides integrated support to people experiencing complex health and social issues related to substance use and addictions: and aims to connect people who are experiencing homelessness or are at risk of losing their housing and/or people who have mental health, addictions and substance use health needs to treatment and preventative services, while addressing basic needs such as food, clothing and showers.

What You’ll Do

The Nurse Care Coordinator plays a central role in supporting clinical operations and ensuring high-quality, coordinated care within the HART Hub—an interdisciplinary program serving individuals with complex health and social needs. This role involves working closely with the HART Hub team to promote integrated, client-centered care across multiple SWCHC sites and services.

In the initial phase, the Nurse Care Coordinator will focus on supporting clients entering the supportive housing program, collaborating closely with the Nurse Practitioner to help ensure successful transitions and sustained tenancies. This role will continue to evolve as the program develops.

Key responsibilities include clinical coordination, supervision of nursing staff, crisis intervention, and contributing to policy and program development. The Nurse Coordinator also plays a key role in quality improvement initiatives, supports compliance and reporting activities, and works collaboratively with the HART Hub leadership team to identify service gaps and guide program priorities.

The Nurse Care Coordinator will have an active outreach component, engaging with clients in local rooming houses within the Somerset catchment. They will work alongside the HART Hub team to identify and support individuals who may benefit from the supportive housing model. Additionally, the Nurse Coordinator will contribute to the development of medical directives that allow the nursing team to work autonomously within their scope.

What We’re Looking For

* Life and Work Experience

• Minimum 3 years of nursing experience in mental health and addictions; preference given to candidates with withdrawal management or addiction medicine experience.
• At least 1 year of case management
• Minimum 1 year in a primary care or community health
• Minimum 1 year in an acute, urgent, or emergency care
• 1–3 years of leadership or supervisory
• Experience in program development, including identifying service gaps, implementing new initiatives, and evaluating outcomes.
• Experience integrating supportive housing and medical services, with an understanding of how to coordinate care in housing-based environments to improve health and stability outcomes.
• Experience working with vulnerable and marginalized populations.
• Demonstrated commitment to health equity and anti-oppressive practice.
• Priority consideration for candidates identifying as Black, Indigenous, or from other equity-deserving groups.
  

* Your Education Path

• Baccalaureate Degree in Nursing (or higher).
• Current registration with the College of Nurses of Ontario (CNO).
• Professional Liability Protection (PLP).
• Current CPR Level C
• Successful completion of a police record check for vulnerable populations.

* Language Requirements

• Fluency in English (spoken and written) is required.
• French or other languages are considered strong assets.

What You Bring to the Table

• Strong clinical knowledge of harm reduction, mental health, substance use, and trauma-informed care.
• Experience managing opioid agonist therapy (OAT), withdrawal symptoms, and overdose prevention.
• Skilled in non-violent crisis intervention, de-escalation, and emergency response.
• Familiarity with RNAO Best Practice Guidelines and clinical application.
• Advanced assessment and decision-making skills in complex care scenarios.
• Ability to lead interdisciplinary teams and support staff development.
• Proven ability to develop care plans and navigate complex systems of care.
• Excellent organizational, interpersonal, and communication skills.
• Comfortable with data analysis, reporting, and contributing to research initiatives.

Your Day-to-Day at a Glance

• Coordinate care for clients living in supportive housing, rooming houses, and/or shelters.
• Participate in the recruitment, onboarding, coaching, and mentoring of nursing staff and allied health professionals, including Community Care Workers.
• Support daily clinical operations and respond effectively to shifting program needs.
• Provide crisis intervention, including overdose response and medical emergencies.
• Collaborate with team members to develop and implement individualized care plans.
• Ensure compliance with clinical standards, regulatory requirements, and funding agreements.
• Identify service gaps and contribute to program development through active engagement in Communities of Practice and inter-agency collaboration.
• Support the development and implementation of clinical policies and quality improvement initiatives.
• Provide clinical guidance and oversight to the interdisciplinary care team.
