Position Overview:

  • Performs patient assessments to identify care needs, documents findings, and consults with the interdisciplinary team.
  • Executes procedures to implement care plans based on standards and protocols.
  • Develops and revises care plans, orders tests, reviews results, and discusses plans with patients and families.
  • Documents all care activities, evaluates outcomes, and contributes to performance improvement.
  • Implements practice initiatives, coordinates care to optimize outcomes, and facilitates patient education and communication among departments.
  • Leads quality improvement processes, analyzes practices, and revises standards based on research and best practices.
  • Mentors team members, seeks leadership opportunities, and develops educational plans.
  • Engages in self-development and provides staff educational programs.
MINIMUM REQUIREMENTS

Physician Assistant

  • Graduate of an accredited physician assistant program, NCCPA certified.
  • Preferred: One-year experience in primary or acute care.
  • NJ Physician Assistant License.

Nurse Practitioner

  • Bachelor's in nursing and Master's in advanced practice or post-Master's certificate.
  • Preferred: One-year experience in primary or acute care.
  • Advanced Practice Nurse – NJ License.
PHYSICAL DEMANDS AND WORK ENVIRONMENT
  • Frequent: Sitting, Standing.
  • Occasional: Walking, Climbing, Carrying, Pushing/Pulling, Twisting, Bending, Reaching, Squatting/Kneeling/Crawling, Wrist deviations, Fine motor activities, Keyboard use, Talking, Hearing.
  • Continuous: Taste, Smell.
  • Lifting: Up to 20 lbs. floor to waist; no lifting above waist.
  • Sensory: Near and far vision, color discrimination, depth perception, hearing.
