North Simcoe Community Support Navigator
Part-Time (Contract)

POSITION OVERVIEW
The Part-Time Community Support Navigator (CSN) plays a key role in supporting individuals, families, and caregivers in North Simcoe by facilitating timely, person-centered connections to local health, social, and community services. The CSN works under the guidance of the Lead Community Support Navigator and collaborates closely with Georgian Bay General Hospital (GBGH), Ontario Health atHome, and other providers to enhance client care transitions and access to supports.

This position ensures that individuals are linked to the right services at the right time—promoting independence, safety, and overall well-being within the community.
POSITION DETAILS
Contract Term: Until April 3, 2026
Positions Available: 2 part-time roles
Rotational Schedule: Seven (7) days on / seven (7) days off
Compensation: Scheduled hours will be compensated at an hourly rate commensurate with skills, experience, and qualifications. Standby coverage will be compensated at a flat rate of $7.00 per hour, with additional pay for any active service provided during standby periods.

Scheduled Hours (Office-Based)
Monday to Friday: 1:00 PM – 4:00 PM
Location may alternate between the ILS Simcoe County Midland office and/or Georgian Bay General Hospital (GBGH) based on need.

Standby Coverage – GBGH Emergency Department Only
Monday to Friday: 4:00 PM – 7:00 PM
Saturday & Sunday: 11:00 AM – 7:00 PM
No in-office presence required during standby hours
Must be within 20-minute response time of GBGH
Direct service delivered during standby hours will be compensated at an hourly rate, based on actual time worked.


KEY DUTIES AND RESPONSIBILITIES:
1. Client Navigation and Referral Coordination
  • Engage directly with individuals, caregivers, and healthcare providers to assess needs and determine eligibility for community support programs.
  • Provide informed referrals to services including personal support, assisted living, transportation, food access, housing supports, and social programs.
  • Follow up with clients and providers to confirm service access, support barrier resolution, and ensure continuity of care.
2. Hospital and Community Collaboration
  • Collaborate with GBGH discharge planners and emergency department staff to identify clients in need of post-discharge support.
  • Support timely discharge planning by coordinating appropriate services prior to or following hospital release.
  • Work with Ontario Health atHome Care Coordinators and other regional partners to align care plans and services.
3. Documentation and Data Management
  • Document all referrals, interactions, and follow-up actions using ILS Simcoe County’s client management database (GoldCare).
  • Maintain accurate records to support outcome tracking and performance evaluation.
  • Ensure compliance with organizational policies and relevant privacy legislation.
4. Community Engagement and Resource Awareness
  • Maintain up-to-date knowledge of local services, eligibility requirements, and referral procedures.
  • Assist with the development or updating of resource directories and service maps.
  • Represent the CSN role at community events or engagement activities when needed.
5. Team Collaboration and Communication
  • Work collaboratively with the Lead CSN and peer Navigators to ensure coordinated service delivery across the North Simcoe region.
  • Participate in regular team check-ins, training, and meetings to support ongoing development and quality improvement.

QUALIFICATIONS

Education/Experience
  • Post-secondary education in social work, community services, health sciences, gerontology, or a related field preferred
  • Minimum 2 years’ experience in a client-facing or navigation role within health or social services
  • Knowledge of local community services and healthcare system an asset

Skills & Competencies
  • Strong interpersonal and communication skills, with a client-centered approach
  • Ability to work both independently and as part of a team
  • Proficient in documentation, case coordination, and time management
  • Comfortable with technology and data entry (e.g., electronic records, databases)
  • Proven ability to handle sensitive information with discretion and maintain a high level of confidentiality.

Working Conditions
  • This position requires some travel within the North Simcoe region. A valid driver’s license, insurance and access to a reliable vehicle required.
  • A combination of in-office, some virtual work, and community-based activities will be expected.
  • Availability for standby shifts and within required response times
