Job summary

WoodGreen Community Services is seeking a Personal Support Worker for night shifts (Mon-Fri, 11pm–7am) to provide in-home and site-based support to clients within Community Care. This on-site role at 1080 Queen Street East requires a PSW with a compassionate approach and strong teamwork skills.

The successful candidate will have a PSW certificate (NACC where applicable) and at least one year of experience delivering personal care to seniors and disabled clients.

Qualifications

• Minimum one year experience providing personal care to seniors and people with disabilities.
• Personal Support Worker certificate and NACC certificate (if applicable).

Responsibilities

• Provide care to clients in their homes or approved sites as scheduled.
• Follow care plans and document client progress for Supervisor review.
• Support activities of daily living including mobility, hygiene and meal preparation.
• Maintain safety through proper lifting, transfers and use of equipment.
• Record and report any changes in client condition promptly.
