Responsibilities include developing client-centered case management plans, supporting relationships with natural supports, setting goals, resolving crises, accessing mental health and addiction services, securing housing, fostering cultural connections, navigating systems, and maintaining employment or education. The role involves building community relationships, documenting casework, data collection, and reporting outcomes.
