Job description

Job Purpose

The Social Worker within the MAID program works collaboratively with a multidisciplinary team within the Community Health Centre and partners with many groups such as government organizations, non-profit organizations, the private sector, and a multitude of community groups and other community organizations. The Social Worker helps to identify and improve the social determinants of health that impact the overall wellness of individuals, families, and the community, such as income, education, social environment, community inclusion, culture, health practices, employment and working conditions. The Social Worker uses a population health approach through individual supportive counseling, group work, education, health promotion, advocacy, chronic disease management and linking people to community resources and helping them navigate systems. The work is based in client and family centered care.

Qualifications

  • A university degree in Social Work. Must be registered and a member in good standing with the New Brunswick Association of Social Workers.
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.
  • Non-Violent Crisis Intervention or obtain within 1 year.
  • Care of the dying patient and family.
  • Possess a valid driver's license and reliable transportation, with the ability and willingness to travel within the region to support patient care, attend client appointments and participate in community-based activities as required. Possess a valid passport to work in Area 2.

Reporting Relationship

  • Reports to the Regional Director of Nursing Practice.
