A health organization in New Brunswick is seeking a Social Worker to work in the MAID program, collaborating with a multidisciplinary team. Key responsibilities include assessing and providing supportive counselling, maintaining clinical records, and participating in care planning. The ideal candidate must have a degree in Social Work and be registered with the New Brunswick Association of Social Workers.
Job Purpose
The Social Worker within the MAID program works collaboratively with a multidisciplinary team within the Community Health Centre and partners with many groups such as government organizations, non-profit organizations, the private sector, and a multitude of community groups and other community organizations. The Social Worker helps to identify and improve the social determinants of health that impact the overall wellness of individuals, families, and the community, such as income, education, social environment, community inclusion, culture, health practices, employment and working conditions. The Social Worker uses a population health approach through individual supportive counseling, group work, education, health promotion, advocacy, chronic disease management and linking people to community resources and helping them navigate systems. The work is based in client and family centered care.
