Job summary

Stonehenge Therapeutic Community in Ontario seeks a Registered Practical Nurse to support housing and clinical care for individuals with addictions and mental health needs. The role covers bio-psycho-social-spiritual care, health assessments, crisis intervention, medication management, and collaboration with a multidisciplinary team.

The position is mobile within the Guelph-Wellington area, requires a valid Ontario driver''s license, and may include evenings/extended hours.

Qualifications

• Practical Nursing Diploma from a CNO-recognized program.
• Registered Practical Nurse in good standing with the College of Nurses of Ontario; RPNO an asset.
• Evidence of professional practice liability insurance; valid Ontario driver’s license required.
• Two years in addictions or mental health; withdrawal management experience preferred.
• Satisfactory background checks and vaccination requirements may apply.

Responsibilities

• Assess, plan, implement, and evaluate bio-psycho-social-spiritual care needs per best practices.
• Conduct health assessments including crisis levels, risk to self/others.
• Manage substance use disorders and co-existing conditions; use validated scales.
• In live-in settings, manage medications and liaise with pharmacies; document accordingly.
• Provide health education and participate in team meetings and care planning.
