Job summary

Stonehenge Therapeutic Community in Ontario seeks a Registered Practical Nurse for RAAM to join a multidisciplinary team delivering short-term addiction care. You will provide direct patient care, assessments, crisis intervention, and health education in-clinic and mobile settings across Guelph-Wellington.

Ideal candidates are legally licensed RPNs in good standing with CNO, with 2+ years in addictions or mental health, strong documentation, and willingness to travel regionally.

Qualifications

  • Practical Nursing Diploma from a CNO-recognized program.
  • Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO) required; Member of RPNO an asset.
  • Evidence of Professional practice liability insurance (minimum $2 million)
  • Current Ontario driver’s license with reliable transportation
  • Vulnerable Sector Screening check required pre-employment; PWSG Clearance may be required
  • Valid First Aid and CPR certification required pre-employment
  • May be required to meet occupational health and policy requirements including COVID-19 vaccination

Responsibilities

  • Assess, plan, implement, and evaluate care needs per best practices; document electronically.
  • Conduct health assessments, evaluate emotional, cognitive, and behavioural states, including crisis levels.
  • Assess and manage substance use disorders and co-existing conditions; complete screening tools.
  • Complete physical health assessments and focused interventions (e.g., wound care).
  • Provide nursing interventions and treatments as ordered; respond to medical emergencies.
  • In live-in settings, manage client medications with pharmacy liaison and documentation.
  • Provide health-related education to clients and families.
  • Collaborate in interdisciplinary case conferences and care planning.
  • Contribute to program development, policy updates, and quality improvement initiatives.
  • Support the RAAM and mobile clinic operations, including travel within Guelph-Wellington.

Job description

Stonehenge Therapeutic Community provides a range of residential and community-based treatment services to individuals struggling with addiction and concurrent mental health issues. About the Role:

The Rapid Access Addiction Medicine Clinic (RAAM) is a specialized medical clinic designed for people who are experiencing health issues related to substance use. The services provided through RAAM clinics are intended as short-term interventions, and participant are connected back to their primary care provider upon completion of their involvement. This program is provided both in person and virtually to those residing in Waterloo and Wellington Counties.

The Registered Practical Nurse – RAAM will work collaboratively with a multi-disciplinary team, including Nurse Practitioners, peers and Counsellors dedicated to improving health outcomes for individuals impacted by substance use. RPN staff provide direct patient care, working within the full RPN scope of practice

Location and Hours of Work Information:
  • Monday to Friday, days (typical hours are 8:30am-4:30pm).
  • One (1) Position Available
  • Work will be assigned on a rotational basis supporting RAAM clinics in Guelph, and Fergus as well as through our Mobile RAAM clinic which provides mobile support within Guelph-Wellington. RPN staff will also work out of STC office locations in Guelph. 1 based out of Guelph. Outreach support will be provided on a regular basis, requiring staff to travel within Guelph-Wellington
Qualifications:
  • Practical Nursing Diploma from a CNO-recognized program
  • Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO) required; Member of RPNO an asset
  • Evidence of Professional practice liability insurance (minimum $2 million)
  • Minimum two years of progressive experience in addictions or mental health; Experience in inpatient or community setting preferred
  • One year of recent experience in withdrawal management preferred
  • Experience in concurrent disorders or trauma informed practice an asset
  • Knowledge of suicide prevention and crisis intervention strategies
  • Lived experience with addictions and/or mental health an asset
  • Valid Ontario Driver’s License, with reliable (own) transportation
  • Satisfactory Motor Vehicle Record
  • Proof of valid Ontario driver’s license (G2 or G), with a minimum of 3 years of driving experience
  • Vulnerable Sector Screening check required pre-employment; PWSG Clearance may be required at the discretion of the Agency
  • Satisfactory reference check required pre-employment
  • Current First Aid and CPR certification required pre-employment
  • May be required to meet occupational health and policy requirements of partner Agency locations including full COVID-19 vaccination, annual influenza vaccination and full immunization series
Role Overview:
Direct Client Support
  • Assess, plan, implement, and evaluate bio-psycho-social-spiritual care needsin accordance withbest practices; document electronically and/or manually.
  • Conduct health assessments to evaluate emotional, cognitive, and behavioural states, including crisis levels, aggression, self-harm, suicide risk, risk to others
  • Assess and manage clients with substance use disorders and co-existing conditions; complete screening tools for mental health, screening for sexually transmitted and blood borne infections.
  • Assess and manage substance use and substance use disorders including completion of validated scales and reviewing diagnostic criteria
  • Complete physical health assessments and focused physical health interventions e.g. wound care
  • Provide nursing interventions and treatments as ordered
  • Respond to medical emergencies and initiate appropriate crisis interventions
  • Collect specimens (i.e. blood, urine, blood glucose) as required
  • In live-in settings, assist with management of client medications including liaising with community pharmacies, medication record reconciliation administration and documentation
  • Identify learning needs and provide health related education
Collaboration & Documentation
  • Complete all documentation in accordance with STC policies and the College of Nurses of Ontario (CNO) standards
  • Participate in interdisciplinary case conferences and contribute to collaborative care planning
  • Participation in regular team meetings
  • Provide input into program development, policy updates, and quality improvement initiatives
  • Perform additional related duties as assigned to support program operations and client care
  • Provide support to Supportive Housing RPN team (holiday coverage)
