The salary range for this position is CAD $42.27 - $52.81 / hour

Job Summary

We are looking for a Full Time MHSU Clinician, Bachelors - Community Mental Health & Substance Use Services to join our team in Abbotsford, B.C.

What We're Looking For:
  • Qualifications: Bachelor's degree in Social Work, Clinical Counselling or other relevant health-related discipline. Eligible for registration or membership with applicable professional association or regulatory body.
  • Experience: At least two (2) years' recent, related experience working with clients and families with, or affected by, mental illness and substance use disorders, or an equivalent combination of education, training, and experience.
  • Additional Requirements: Valid Class 5 BC Driver's License and access to a personal vehicle for business-related purposes.

Detailed Overview

In accordance with Fraser Health's Vision, Mission, and Values, program mandates, and clinical practice guidelines, with an emphasis on recovery and rehabilitation and reducing barriers to re-enter the system of care, supports individuals and families affected by mental illness and substance use disorders. Focuses on advocacy, supportive counselling, psycho-education, community resourcing, and care coordination within the local service area, ensuring timely and relevant treatment, rapid re-engagement, and access to information. Services may be provided on an outreach basis to assess and engage clients in their environment and community.

As a clinician in a shared-team model, provides intake, screening, assessment, treatment planning, referrals, follow-up support, group education, community resource liaison, and acts as the 'most responsible clinician' for specific clients based on their needs. Provides trauma-informed, client and family-centered care, encourages community engagement, and supports navigation of the healthcare system.

Responsibilities
  1. Develop and implement recovery-oriented treatment plans collaboratively with clients and supports; provide case consultation and support discharge and transition planning; update the Clinical Coordinator on client progress and significant issues.
  2. Provide individual, group, and family support using various assessment and counselling methods; share workload within the team for case management, crisis intervention, home visits, group facilitation, and outreach.
  3. Assist with intake, assess client needs, gather background information, and determine care levels and priorities, collaborating with the interdisciplinary team and addressing emergent crises.
  4. Facilitate client access to community services and programs, including housing, therapy, and other rehabilitative services; make referrals and liaise with providers and community resources.
  5. Advocate for clients, providing information about appeal processes, completing forms, and acting as a resource for clients and families.
  6. Arrange hospital admissions when necessary, including involuntary admissions, coordinating with healthcare providers and transportation.
  7. Link clients with community and acute care services for discharge planning, liaising with forensic and other specialized programs.
  8. Conduct individual and group sessions, including psycho-education and skill development, involving families and community activities to promote knowledge exchange.
  9. Maintain accurate client records, ensuring confidentiality and compliance with policies and regulations.
  10. Provide follow-up services, including referrals to other professionals.
  11. Participate in quality improvement, risk management, ongoing education, and community committees; contribute to clinical research and program evaluation.
  12. Perform other duties as assigned.
Qualifications
Education and Experience

Bachelor's degree in Social Work, Clinical Counselling, or relevant health discipline. Two (2) years' recent, related experience, or an equivalent combination. Eligible for professional registration. Valid BC Driver's License and access to a vehicle.
