A community health service organization in Toronto seeks a Mental Health Worker to provide support and assessments for clients dealing with mental illness and addictions. Applicants should have a degree in social services and two years of experience in direct services. Reporting to a clinical program manager, the Mental Health Worker will provide on-site assessment, intake, discharge planning, and support, as well as liaising with existing supports, assisting with goal setting, and planning for clients in West Toronto.
