Experience You Bring & Qualifications
- Master of Counselling Psychology, Clinical Counsellor, Registered Nurse with Mental Health experience from an accredited school or body
- Passion for helping individuals navigate mental health challenges to support overall health and well-being
- Case management expertise, cultivating client relationships for optimal outcomes
- Experience using an electronic medical record (EMR) system and/or Microsoft 365 considered an asset
- At least one year of clinical experience
The Role: Mental Health Care Navigator
- Assessing the clients’ emotional and mental health needs in a case-management setting through interviews, identifying concerns and screening results of mental health related interventions both internally and externally.
- Collaborating with clients to customize third party treatment options that may include individual counselling, group therapy, assessments, community supports or resources.
- Helping clients navigate through different stages of change and promoting client engagement for the recommended mental health interventions.
- Working collaboratively within the interdisciplinary team of healthcare professionals to create meaningful client care, including liaison and case management tasks.
- Building a network of public and private mental health professionals and external treatment resources for the company.
- Educating clients on the Company’s “n of 1” approach to clinical care including precision medicine, pharmacogenetics, and differential diagnosis in mental health.
