Detailed Overview

Reporting to the Program Coordinator or designate and under the direction of the team leader, the Licensed Practical Nurse (LPN) Outreach works as a member of the multidisciplinary mental health services team in a collaborative nursing model with RNs and RPNs. The LPN Outreach performs assessments and plans, implements, and provides personal and nursing care services to clients, both in their homes and in the community, who have various mental health disorders. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), adhering to Fraser Health policies, procedures, standards, and philosophies.

Responsibilities
  1. Administers oral, subcutaneous, and intramuscular medications according to pharmaceutical and organizational policies.
  2. Assesses and monitors clients' physiological, psychological, socio-cultural, and spiritual needs regularly.
  3. Provides a supportive environment; contributes to and modifies clients' care plans; participates in interdisciplinary care planning meetings.
  4. Establishes care priorities, responds to changing client needs within scope and competency guidelines.
  5. Delivers nursing care in clients' homes and community settings, evaluates outcomes, records observations, and reports issues.
  6. Documents care procedures to ensure patient safety.
  7. Assists clients with activities of daily living, including meals, bathing, dressing, and skin care; oversees exercise routines.
  8. Participates in quality improvement initiatives and team collaboration activities.
  9. Maintains a safe environment, reports unsafe conditions or equipment issues.
  10. Supports orientation and education of new staff and students in collaboration with management.
  11. Performs other duties as assigned.
Qualifications

Education and Experience

Graduation from a recognized Practical Nursing program or equivalent; current registration with BCCNM; current First Aid, CPR, and Food Safe certifications; completion or eligibility for MHSU Competency Education; valid BC Driver's License and access to a personal vehicle with insurance.
