The Manager, Operations and Integrated Services will provide operational management of an integrated health and community service initiative - the Primary Care Action Team (PCAT) program. Reporting to the Director of Community Health or designate and working collaboratively with stakeholders across Flemingdon Health Centre, Ontario Health Teams and Primary Care Networks, the Manager will ensure the development and implementation of a high quality, integrated, interdisciplinary primary health care program. This position includes integration leadership, such as stakeholder management and system level strategy, operations leadership, such as project management and budget monitoring, with implementation services leadership, such as program delivery and managing staff.
This position focuses on leadership within a collaborative, client-centred and equity-focused framework. The successful candidate will be skilled in stakeholder engagement, project management best practices, and healthcare transformation.

• Bachelor's degree in health administration, Public Health, Business Administration, or related field.• Project Management Certification (PMP or equivalent) considered an asset.• Minimum 3 years of experience managing or coordinating complex projects in health care, social services, or related sectors.• Experience developing and/or implementing primary care programs• Strong organizational skills and experience developing and maintaining project documentation.• Demonstrated experience with stakeholder engagement and cross-sector collaboration.• Knowledge of the Ontario health care system and/or experience in integrated care models. Experience applying quality improvement frameworks and tools is considered an asset.• A clinical background is considered an asset.• Excellent interpersonal, written, and verbal communication skills.• Proficient with Microsoft Office Suite, project tracking software (e.g., MS Project, Asana, or equivalent) and Office 365.
