Manager of Community Support Services (CSS)

The Job Summary
As a member of Community Support Services, a multidisciplinary team, the Manager, Community Support Services (CSS) role is to provide leadership and guidance to the team members to ensure that the highest quality of support and services are provided to participants and care partners. The Manager, CSS will be working alongside the Director of Community Support Services and provide support with day-to-day operations on all SJHCG CSS sites/ program spaces, and clinical leadership within the Community Support Services (CSS) Department. The Manager, CSS is accountable to the staff and clients, and reports to the Director of Community Support Services
Key Accountabilities & Success Criteria
- In consultation with the Director, the Manager will manage comprehensive therapeutic Day Programs for the frail elderly, individuals with Alzheimer Disease and/or a related dementia and survivors of Acquired Brain Injury.
- Actively participate in the recruitment, orientation and retention of high performing staff
- Review, allocate and approve vacation time, lieu time and educational leaves, and other leave of absences.
- Review and approve weekly payroll and schedules
- Support Director CSS with developing annual budget, monitoring monthly variance reports and developing balancing strategies.
- Support Director CSS with bi-weekly, monthly, one-time, quarterly, and annual reporting requirements.
- Assign staff specific caseload responsibilities.
- Develop and revise staff schedules and delegate responsibilities
- Meet regularly with the Director of Community Support Services to plan, problem solve, implement and evaluate service provisions.
- Assist the Director of Community Support Services to operationalize department budget, while being fiscally responsible and maintaining safe operating procedures.
- Maintenance of balanced budget, and inventory, capital requests and purchase
- Review, approve, track and follow up on reassessments on fee subsidy requests
- Design and implement programs that align with the current needs of the target population.
- “Trouble shoot” operational challenges and concerns for staff, participants, families, and physicians.
- Promote and support ongoing staff professional development, and core competencies by positions
- In collaboration with Supervisor and Clinical Leads, the Manager, CSS will coordinate and complete staff performance appraisals.
- Initiates corrective actions related to occupational health and safety, works with WSIB claims, and participates in return-to-work planning.
- Liaise with HR and union as needed when progressive disciplinary planning is necessary.
- Performance management of direct reports as needed
- Support and implement the development of policies, processes and procedures that meet best practice guidelines in adherence with professional standards of practice.
- Ensure participant and staff safety by identifying unsafe conditions or practices and take appropriate action to address them including reporting or removing hazards.
- Develop and foster positive working relationships with other community agencies for coordination of care and program planning.
- Collaborate with the Director of Community Support Services to conduct annual program evaluations and compile reports that inform quality improvement initiatives.
- Chair and/or attend departmental, organizational, regional, provincial, and system meetings as required
- Represent the Day Programs on appropriate committees within St. Joseph's Health Centre Guelph and in the community.
- Lead and facilitate annual strategic planning and goal development for CSS
- Coordinate the development and management of continuous improvement activities within the department and facilities as part of the Quality Improvement Plan (QIP).
- Promotes an environment that encourages and supports change using change leadership theory.
- Support Director CSS in all accreditation activities, data collection, update processes and lead the department in engagement exercises.
- Assumes 24-hour on-call availability for consultation and/or reporting purposes as required.
- Accountable for reviewing client RL Solutions and sign-offs in addition to employee incident events and processes.
- Supervise clinical practice in the Day Programs and coordinate regular team meetings.
- Oversee audits of clinical documentation.
- Act as advocate on behalf of participants and caregivers, when needed.
- Consultation with colleagues regarding participant and caregivers’ needs are beyond the scope of existing SJHCG CSS eligibility and suitability criteria
- Consult with Clinical Lead/Clinical Resource worker with regard to case conferences and attend, as appropriate.
- Provide consultation to Clinical Team regarding management of behavior challenges and development of care management strategies, and crisis intervention, as needed.
- Contributes to a transparent culture of client and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by SJHC.
Qualifications
- Undergraduate degree in the field of Health or Social Sciences with an emphasis in Gerontology, Recreation Therapy, Human Kinetics or Kinesiology, Nursing, BScN Degree.
- Minimum five (5) years of experience in Community support sector.
- Minimum 3-5 years of management experience in a health-related field preferred.
- Experience working in a Unionized environment is preferred.
- Geriatric Certificate, GPA, Dementia Care, Lean Training, Mental Health First Aid preferred.
Skills & Abilities
- Strong communication, leadership, interpersonal, conflict management and problem-solving skills.
- Identify gaps in services and provide opportunity for growth at a department/system level.
- Strong risk assessment skills and ability to implement risk mitigation strategies as needed.
- Collaborative leadership approach including ability to motivate, coach and provide capacity building amongst team members.
- Strong business and financial acumen for fiscal planning and budgeting
- Valid Drivers license with regular access to a vehicle- travel will be required within Waterloo Wellington Region
