SKILLS:

* Strong communication, consultation and interpersonal skills

* Highly motivated, people oriented professional

* Minimum of 5 years of supervisory experience

* Demonstrated knowledge of social gerontology and general geriatric care.

* Demonstrated knowledge of services for targeted client population

* Bilingual – English/French language is an asset

* Demonstrated skill in the use of MS Office Suite

* Demonstrated ability to function in a team environment

* Demonstrated ability to work independently

* Ability to represent Huron Lodge in a courteous and professional manner

* Valid driver’s license

SUMMARY OF DUTIES:

Responsible to the Chief Executive Officer, through established lines of authority, for planning, leading, organizing and controlling of all aspects of the daily operation of Ministry funded Assisted Living for High Risk Seniors program at Huron Lodge. Provides leadership through common vision, commitment and understanding of clients’ needs. Assesses requirements and adapts programming for clients of the Assisted Living for High Risk Seniors program.

Reports and responds to problems affecting the delivery of Assisted Living for High Risk Seniors program and seeks solutionsto correct performance deficiencies within those functions.

Reviews and updates policies and procedures for Assisted Living for High Risk Seniors program and communicates to staff.

Assists with research related to best practices in Assisted Living for High Risk Seniors program.

Cooperates with other departments and staff within the organization.

Supervises, trains and schedules all Assisted Living for High Risk Seniors staff.

Coordinates and ensures the effective delivery of the Assisted Living for High Risk Seniors program and services.

Performs initial, in-home client needs assessments and determines suitability according to the various goals and criteria of the programs.

Ensures the maintenance and confidentiality of information and records for Assisted Living for High Risk Seniors clients.

Maintains contact and monitors well-being of clients to ensure adequacy of care and to ascertain the need for additional or modified services.

Promotes activation and socialization programs for clients.

Liaison with family members of Assisted Living for High Risk Seniors clients as required.
