Job Summary
Provides nursing care to patients under the supervision of a Registered Nurse or physician/advanced practice provider. Uses observational skills to monitor nursing interventions and assists medical staff to ensure optimal care within established policies, procedures, and standards. Performs nursing duties within the legal scope of practice, demonstrating performance aligned with the organization’s Mission, Vision, and Strategic Plan. Treats coworkers, patients, families, and all healthcare team members with dignity and respect.
Essential Responsibilities
- Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; conforms to department service standards.
- Provides therapeutic patient care as established by physicians/advanced practice providers and/or registered nurses within scope of practice.
- Transcribes physician/advanced practice provider orders following policy.
- Communicates with registered nurses to modify care plans; assists in developing nursing care plans.
- Administers specified medications accurately per provider orders within scope of practice.
- Performs treatments such as urinary catheterizations, IV placements, irrigations, suctioning, throat irrigations, and enemas, adhering to infection control policies.
- Ensures cleanliness of nursing stations, patient rooms, and proper functioning of equipment to maintain a safe environment.
- Performs related patient care functions: taking vital signs, wound care, patient education, specimen collection, etc.
- Assists with examinations and treatments, supporting medical staff as directed.
- Documents patient condition and responses to therapy; initiates emergency procedures if needed.
- May start IV therapy and perform venipuncture with certifications.
- Supports patient education and discharge planning.
- Participates in staff meetings, training, and maintains supplies and equipment.
- Supports communication with patients via various channels.
- Documents care accurately in Health Connect per policies.
- Performs other duties as assigned, including orientation of new staff and discharge coordination.
- Maintains compliance with confidentiality, privacy laws, and organizational policies.
- Utilizes skills assessments to ensure proficiency in caring for specific patient age groups.
Experience & Education
- Minimum of 12 months of LVN experience in direct patient care within the last three years, including medical office, acute care, home care, or skilled nursing.
- High School Diploma or GED required.
