Position Description

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies, and applicable regulations.

Qualifications

• Long-term care experience
• Point Click Care charting experience
• Current LVN/LPN license in practicing state
• Current BLS/CPR certification or willingness to acquire one
• Experience in nursing management is preferred but not required

Responsibilities

• Lead daily functions of nursing assistants following rules, regulations, and guidelines
• Ensure compliance with policies and procedures
• Manage resident admissions, transfers, and discharges
• Document patient condition changes and incidents
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction
• Monitor and document patient health status and changes
• Utilize the Quality Life Manual for patient care planning
• Communicate room changes, LOA, admissions, and discharges to relevant departments
• Make rounds with physicians as necessary
