The Role : As a Licensed Practical Nurse (LPN) in Residential Care, you will deliver high-quality nursing care to residents in a long-term care or assisted living environment. Your focus will be on promoting residents' well-being, dignity, and independence while collaborating with a multidisciplinary team to ensure optimal care outcomes.
Responsibilities :
Provide direct nursing care, including administering medications, wound care, and monitoring residents' health status.
Assist residents with daily living activities, ensuring comfort and dignity in care delivery.
Develop and implement individualized care plans in collaboration with the healthcare team.
Respond promptly to changes in residents’ health and escalate concerns to the appropriate healthcare professionals.
Document assessments, interventions, and outcomes in compliance with regulatory and organizational standards.
Foster a safe, compassionate, and inclusive environment that supports residents’ physical, emotional, and social needs.
Requirements :
Current registration as a Licensed Practical Nurse in your province or territory.
Experience in residential care, long-term care, or geriatrics is preferred.
Strong knowledge of chronic disease management, medication administration, and palliative care principles.
Excellent interpersonal and communication skills to engage with residents, families, and team members.
Ability to adapt to residents’ changing needs and work collaboratively in a team-focused setting.
