Responsibilities

The Home Health Patient Care Manager Assistant (Licensed Practical Nurse, LPN) assists the Patient Care Manager in coordinating a team of staff to ensure high-quality care to home health patients in the assigned area.

  • Ensures the team provides quality care efficiently, following physician's orders and State/Medicare guidelines.
  • Prepares patient lists and necessary documents for weekly case conferences.
  • Provides direct patient care as needed, within scope of practice, care plans, and physician orders, in coordination with the healthcare team. May not alter the plan of care.
  • Assists the Patient Care Manager in clinician documentation review/audits to ensure complete, appropriate, and accurate documentation by field staff.
Education and Experience

License Requirements

  • Current CPR certification required.
  • Current LPN licensure in the state of practice.
  • Valid Driver's License, vehicle insurance, and access to dependable transportation.
