At Sunrise, our Wellness Nurse is responsible for the overall well-being of our residents.
RESPONSIBILITIES & QUALIFICATIONS
Responsibilities:
- Identify ongoing needs and services of residents to promote the highest quality resident care.
- Ensure proper follow-up, participate in evaluations, and document changes in residents' health care services and status.
- Assist Resident Care Director (RCD) in conducting resident evaluations and gathering information at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Conduct monthly wellness visits for all residents.
- Monitor and follow up with residents after falls, accidents, or incidents; document and report as required.
- Maintain all pertinent information in the resident wellness files.
- Communicate with residents’ families or responsible parties to ensure medical needs are met.
- Contact residents’ physicians when necessary or upon family request.
- Ensure weights and vital signs are obtained monthly for each resident.
- Assist in maintaining wellness files according to Sunrise policies and regulations.
- Maintain medical supplies and emergency kits for the community.
- Provide clinical support and assistance to team members as needed.
- Follow infection control practices and promote standards including resident testing and training.
- Handle and dispose of bloodborne pathogens, bodily fluids, infectious waste, and hazardous materials in accordance with universal precautions.
- Monitor medication and treatment profiles to ensure proper administration and documentation.
- Participate in quality improvement activities and understand key regulations such as Resident Rights and assessment processes.
Qualifications:
- Must have a BC LPN license (or RN).
- Graduate of an approved nursing program.
- Current professional license in British Columbia.
- At least one year of experience in a healthcare setting such as home health, assisted living, hospital, or long-term care.
- Knowledge of nursing practices, regulations, and good assessment skills.
- Strong organizational, communication, and problem-solving skills.
- Proficiency in computer applications including Microsoft Office and Sunrise software.
- Availability for all shifts, including mornings, evenings, and overnight, with flexibility for weekends.
