LPN Manager of Supportive Housing

The LPN Manager is responsible for the overall operations of the Society’s Supportive Housing Program for adults with serious mental illness, providing psycho-social rehabilitation and support to residents. The primary location is a 24-unit apartment building (Chesterfield House). The position also supports MHS licensed sites and participates in the Management team. This position is out of scope. Most of the residents are also clients of VCH community mental health teams. It supervises mental health workers and maintenance staff.

Key Duties and Responsibilities

  1. Coordinates, develops, and implements individual recovery/ rehabilitation plans using a client-centered approach. These plans include ADL, medications, mental illness management and social engagement, emergency support plans, i.e., implements the Chesterfield rehabilitation program.
  2. Ensures client documentation including but not limited to wellness/recovery plans, taxes, income assessments, progress notes, incident reporting and any required “tenant” agreements are in place and current.
  3. Assesses all referrals for suitability for programs. Arranges for resident admissions, possible re-admissions, and discharges.
  4. Collaborates closely with the Hope Centre Community Mental Health Team and Home Care and other agencies to support tenants/clients.
  5. Assists with budget development and implements approved budgets.
  6. Hires, coaches and supervises Mental Health Workers as needed in Licensed Care Program.
  7. With Leadership team reviews and revises Policy and Procedures as required.
  8. Establishes and maintains professional contacts with the community and related agencies. Ensure that we are "good neighbours".
  9. Directs Facility operations including undertaking inspections. With Caretaker, develops and implements approved building maintenance and repair plan within budget.
  10. With other Managers, participates in on call rotation for urgent items and emergencies.
  11. Provides supportive Housing services to Marineview Housing’s Licensed sites as required approx. 0.25 FTE.

Qualifications:

Education, knowledge, and experience

  1. Licensed Practical Nurse registration
  2. Two to three years experience with Psycho-Social rehabilitation in community Mental Health or equivalent.
  3. Three years experience with supervision. Working with a collective agreement an asset.
  4. Experience in licensed residential care an asset
  5. Demonstrated knowledge of serious mental health illness and treatments.
  6. Demonstrated knowledge of administration, budget management, scheduling and record keeping.
  7. Knowledge and experience in Facility management and The Residential Tenancy Act.
  8. Physical and mental ability to undertake role responsibilities.
  9. Ability to demonstrate leadership ability including conflict resolution.
  10. Ability to speak another language, particularly Farsi an asset.
  11. Driver’s License and vehicle required.
  12. Current First Aid/CPR, Naloxone, and Food Safe certification required.
  13. Ability to flex time to meet operational needs.
