Job Details

With a commitment to patient- and family-centered care, the LPN uses the nursing process in ways that values the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health. The LPN is prepared to function as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment. Under the direction and supervision of the licensed clinical team member, LPNs carry out selected nursing acts for patients that effectively address the individual needs and priorities of the patient and are consistent with their cultural beliefs and values.


  1. Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson.
  2. Integrates Evidence-Based Practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal healthcare.
  3. Partners with patients, their family/designated care partner(s) in identifying learning needs and priorities and provides education in ways that are useful, understandable, and affirming.
  4. Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.
  5. Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
  6. Creates an environment of belonging and inclusion, demonstrating skills in problem solving and conflict resolution.
  7. Actively participates in practice setting-based committees, including safety and quality improvement.
  8. Demonstrates willingness to give and receive feedback and integrates appropriate changes into practice.
  9. Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision making.
  10. Demonstrates a commitment to learning and seeks opportunities for continued professional growth and development and acts in primary preceptor role to appropriate staff members.
  11. Achieves and maintains compliance with all institutional policies and guidelines related to evidence best practices for patient care.
  12. Minimizes risk of harm to patients and others by following established nursing practices and protocols and individual performance.
  13. Uses data to monitor the outcomes of care processes and uses improvement methods to design and test changes to continuously improve the quality and safety of patient care.
  14. Communicates with a registered nurse, physician, and the patient’s health care team members to seek guidance with patient care.
  15. Collects data as allowed by the licensed practical nurse scope of practice as defined by the Pennsylvania/New Jersey Nurse Practice Act.
  16. Works closely with the registered nurse to assure the implementation of safe, quality care of the emergency department patients.

Job Description
