$26.00 - $28.00/Hour

RESPONSIBLE TO: Adult Day Health Care Director

GENERAL DESCRIPTION

Works closely with the RN in all aspects of registrant care to ensure continuity of care. Responsible for delivering care as directed by the RN, administering medications, performing treatments, and supervising CNAs.

FUNCTIONS
  1. Administer medications and treatments according to physician’s orders and policies, recording appropriately.
  2. Report physical and emotional status changes to RN and ADHC Director.
  3. Check and reorder supplies as needed.
  4. Carry out ADHC and registrant care duties as directed.
  5. Support and adhere to the philosophy, standards, policies, and procedures of St. Luke ADHC.
DUTIES AND RESPONSIBILITIES
  1. Monitor staff compliance with policies and procedures.
  2. Assist in emergency care when necessary.
  3. Provide accurate reports to RN and receive reports from RN.
  4. Transcribe physician’s orders and complete necessary forms.
  5. Supervise and instruct care personnel, including CNAs.
  6. Assist with evaluations of registrant needs and review medical histories.
  7. Ensure proper documentation of vitals, weights, and medication administration.
  8. Maintain medication room and supplies.
  9. Assist activities staff and registrants with activities and care.
  10. Handle emergencies, accidents, and medication errors appropriately.
  11. Maintain confidentiality and participate in quality assurance programs.
  12. Perform other duties as assigned by supervisors.
WORKING CONDITIONS
  • Work in well-lit, ventilated areas with physical activity including walking, standing, bending, lifting.
  • Subject to interruptions, infections, and odors.
  • Engage with registrants, families, staff, and visitors under various conditions.
  • Willing to work beyond normal hours and participate in educational programs.
QUALIFICATIONS
  • Graduate of an approved School of Nursing.
  • Current LPN license in New York.
  • Experience in charge position preferred; geriatrics experience helpful.
  • Understanding of geriatric and MR/DD needs.
  • Willingness to learn and participate in continuing education.
  • Ability to supervise and instruct professionally.
  • Well-groomed, courteous, patient, responsible.
  • CPR certification required.
