Job summary

Extendicare invites applications for a Licensed Practical Nurse to join our team in Canada. You will provide planned, patient-centered nursing care to residents in a long-term care setting, under the direction of the Director of Care or Health & Wellness.

You will participate in care conferences, document plans, and ensure regulatory compliance. We value compassion, teamwork, and ongoing professional development.

Qualifications

  • Current LPN license/registration in the applicable province.
  • Experience in a seniors environment and with unregulated care providers preferred.
  • Education/experience in gerontology, rehabilitation, or long-term care.
  • Strong written and oral communication, critical thinking, and care coordination.
  • Sound clinical knowledge with CPR/First Aid.
  • Current BCLS and First Aid certification required.
  • Computer proficiency and MS Office experience.
  • Vulnerable Sector Check within six months.

Responsibilities

  • Provide direct resident nursing care and medication administration per policies.
  • Document care plans, assessments, and changes accurately.
  • Coordinate with inter-professional team on resident care conferences.
  • Support infection prevention and health & safety programs.
  • Delegate to unregulated care providers and supervise routine tasks.
  • Respond to emergencies and medical changes.

Job DescriptionLicensed Practical NurseMission:Reporting to the Director of Care or Director of Health & Wellness, the Licensed PracticalNurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursingcare in accordance with the provincial standards of care.Key Responsibilities:• To work as a member of the inter-professional team as described by Extendicare’sProfessional Practice Model applying the person centered approach to care;• To complete and ensure resident's clinical care assessments, care/service plans anddocumentation are accurate, up to date and changes communicated as required;• To implement and evaluate the effectiveness of the care/service plans and makechanges as required;• To assist with scheduling of resident's care conferences and participate in conferenceswhere applicable;• To work closely with the Director of Care or Director of Health & Wellness to facilitateresident's move in, move out and transfers;• To provide direct resident care and medication/ treatment administration and documentcare in compliance with regulatory and Extendicare Resident Care standards and policiesand procedures;• To process the orders and safe keep drugs and narcotics and administer in compliancewith all regulatory requirements, scope of practice and Extendicare policies andprocedures;• To facilitate and provide resident/family education;• To plan, coordinate, manage & evaluate the resident's care plan working with theresident/substitute decision maker, physicians, nursing & interdisciplinary team andcontracted service providers;• To coordinate nursing team's resident care assignments to ensure efficient and timelydelivery of services to residents;• To respond to and manage significant changes in resident's condition and or medicalemergencies; provide emergency first aid;• To teach, delegate to, and direct unregulated care providers;• To coordinate emergency response in the absence of Director of Care or Director ofHealth & Wellness/ Executive Director;• To respond to resident/family concerns and ensure appropriate action taken and orconcern communicated to Director of Care or Director of Health & Wellness/ ExecutiveDirector and other departments;• To follow and assist the Director of Care or Director of Health & Wellness in thecoordination, implementation and monitoring of Infection Prevention and Control, Healthand Safety and Clinical Programs and Quality/Safety Plans for the Nursing Department;• To work in a position of trust with vulnerable adults (residents);• To uphold and promote the organization's values and philosophy relating particularly toethics, morality, and integrity as set out in Extendicare’s Code of Conduct;• To complete other duties as assigned.Qualifications:• Current registration as a Licensed Practical Nurse in applicable Province/State;• Experience working in a senior's environment and with unregulated care providerspreferred;• Education and experience in gerontology, rehabilitation, community/complex/long termcare an asset;• Must possess strong written and oral communication, critical thinking, care coordination,time management and customer focus skills;• Must have demonstrated sound clinical knowledge;• Current Basic Cardiac Life Support and First Aid certification required;• Computer proficiency and experience with Microsoft office applications.• Must provide a complete and current (within six months) Vulnerable Sector Check(including a Criminal Background Check) or be willing to obtain one.What Extendicare has to offer:At Extendicare, we believe that working as a team creates an environment that allows us toreach our potential.
