Job summary

Extendicare in Winnipeg, MB, is hiring a Licensed Practical Nurse for part-time duties in our long-term care settings.

You will provide direct resident care, medication administration and care coordination with a collaborative inter-professional team, while adhering to provincial standards and Extendicare policies.

Qualifications

• Current registration as a Licensed Practical Nurse in the applicable Province/State.
• Experience in senior care and with unregulated care providers preferred.
• Education/experience in gerontology, rehabilitation, community/complex/long term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.
• Sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification required.
• Computer proficiency and MS Office applications.
• Vulnerable Sector Check within six months or willingness to obtain one.

Responsibilities

• Work as a member of the inter-professional team applying the person-centered approach to care.
• Complete resident assessments, care/service plans and documentation; keep information up to date.
• Implement and evaluate care plans; make changes as required.
• Assist with scheduling resident care conferences and participate where applicable.
• Coordinate move in/move out and transfers with leadership.
• Provide direct resident care and medication/treatment administration per regulations and Extendicare policies.
• Process orders, safekeep drugs and narcotics; administer per requirements.
• Facilitate resident/family education.
• Plan, coordinate, manage and evaluate the resident’s care plan with the care team and physicians.
• Coordinate nursing team assignments for timely delivery of services.
• Respond to changes in resident condition and medical emergencies; provide emergency first aid.
• Teach and supervise unregulated care providers.
• Coordinate emergency response in leadership absence.
• Respond to resident/family concerns and escalate as needed.
• Support Infection Prevention and Safety and Quality plans for Nursing.
• Uphold Extendicare Code of Conduct and ethics.
• Complete other duties as assigned.

Mission:

Reporting to the Director of Care or Director of Health & Wellness, the Licensed Practical Nurse (LPN)is responsible forproviding planned, patient-oriented, goal-directed nursing carein accordance withthe provincial standards of care.

Key Responsibilities:

• To work as a member of the inter-professional team as describedbyExtendicare’sProfessional Practice Model applying theperson centeredapproach to care;
• To complete and ensure resident’s clinical care assessments, care/service plans and documentation areaccurate, up to date and changes communicated asrequired;
• To implement and evaluate the effectiveness of the care/service plans and make changes asrequired;
• Toassistwith scheduling of resident’s care conferences andparticipatein conferences where applicable;
• To work closely with the Director of Care or Director of Health & Wellness tofacilitateresident’s move in, move out and transfers;

• To coordinate nursing team’s resident care assignments to ensure efficient andtimelydelivery of services toresidents;
• To respond to and manage significant changes in resident’s condition and or medical emergencies; provide emergency first aid;
• To teach, delegate to, and direct unregulated care providers;
• To coordinate emergency response in the absence of Director of Care or Director of Health & Wellness/ Executive Director;
• To respond to resident/family concerns and ensureappropriate actiontaken and or concern communicated to Director of Care or Director of Health & Wellness/ Executive Director and other departments;
• To follow andassistthe Director of Care or Director of Health & Wellness in the coordination, implementation and monitoring of Infection Prevention andControl,Health and Safety and Clinical Programs and Quality/Safety Plans for the Nursing Department;
• To work in a position of trust with vulnerable adults (residents);
• To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out inExtendicare’sCode ofConduct;
• To complete other duties as assigned.

Qualifications:

• Current registration as a Licensed Practical Nurse in applicable Province/State;
• Experience working in a senior’s environment and with unregulated care providers preferred;
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset;
• Mustpossessstrong written and oral communication, critical thinking, care coordination, timemanagementand customer focus skills;
• Must havedemonstratedsound clinicalknowledge;
• Current Basic Cardiac Life Support and First Aid certificationrequired;
• Computerproficiencyand experience with Microsoft officeapplications.
• Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
