Responsibilities & Qualifications

Overnight shifts: 10pm-6am

Responsibilities:
  1. Identify ongoing needs and services of residents to promote high-quality resident care.
  2. Ensure proper follow-up, participate in evaluation and documentation for residents with changes in health care services and status.
  3. Assist Resident Care Director (RCD) in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
  4. Conduct monthly wellness visits for all residents.
  5. Monitor and follow up with residents after falls, accidents, or incidents; document and report as required.
  6. Document all pertinent information in the resident wellness file.
  7. Maintain communication with residents’ families or responsible parties to ensure medical needs are met.
  8. Contact residents’ physicians when necessary or upon family request.
  9. Ensure weights and vital signs are obtained monthly for each resident.
  10. Assist in maintaining wellness files according to Sunrise policies and regulations.
  11. Maintain medical supplies and emergency kits.
  12. Provide clinical support and assistance to community team members as needed.
  13. Follow infection control practices and promote standards including testing, training, and skills checks.
  14. Attend to and dispose of blood-borne pathogens, bodily fluids, infectious waste, and hazardous materials in compliance with universal precautions.
  15. Monitor medication and treatment profiles to ensure accurate administration and documentation.
  16. Participate in quality improvement areas such as Resident Centered Care Model, APIE, and Quality Care Indicators.
  17. Comply with all relevant regulations and uphold Resident Rights and assessment processes.
Qualifications:
  1. Graduate of approved nursing college or school.
  2. Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  3. Minimum of one year experience in relevant healthcare settings.
  4. Knowledge of nursing practices, regulations, and assessment skills.
  5. Effective communication, organizational, and problem-solving skills.
  6. Proficiency in computer skills including Microsoft Office and Sunrise applications.
  7. Availability for weekends, evenings, and flexible hours.
