Manages a designated group of nursing employees on a nursing unit and delegates/supervises resident care.
Administers medications/treatments on the assigned unit.
DUTIES AND RESPONSIBILITIES:
Works closely with the Nurse Manager/Nursing Supervisor in all aspects of resident care to ensure continuity of resident care.
Assumes responsibility for the care delivered on assigned shift, in the absence of the Nurse Manager.
Administers medications and charts the same in the appropriate places.
Performs treatments, etc., and charts the same in the appropriate places.
Supervises and instructs CNA's in the performance of their duties.
Makes rounds to evaluate resident status.
Must be available to periodically work more than eight (8) hours per day, or more than forty (40) hours per week.
Administers medications and treatments in accordance with written physician's orders and established policy and procedures and records the same in appropriate places.
Observes, measures, and records indications of patient health status.
Checks medication/treatment supplies and reorders as necessary.
Carries out unit administration and resident care duties as directed.
Supports and demonstrates practice consistent with the mission, philosophy, standards, policies, and procedures of St. Luke Health Services.
Communicates unit problems to nurse manager/shift supervisor.
Monitors unit staff for compliance with established policies and procedures.
Assists in emergency care as necessary.
Gives concise, accurate, qualitative report from your shift to oncoming Charge Nurse.
Receives report from previous shift.
Transcribes physician's orders and completes all necessary forms (i.e., MAR, lab request, x-ray request).
Assists with orientation, instruction, and supervision of other resident care personnel.
Assists the Nurse Manager/Nursing Supervisor with evaluation of resident's needs, condition, and care.
Reviews resident's medical and social history.
Relieves CNA's for breaks and mealtime when necessary.
Monitors resident's meal times as directed.
Directs and assists with unit cleaning, as well as the care of supplies, equipment, and appliances.
Follows proper policy and procedure with maintenance of controlled substances (i.e., administration, count, documentation of effectiveness).
Cleans and maintains the medication room and cart for the oncoming shift. Also cleans tops and sides of bottles that contain liquid medications.
Reports changes in resident’s condition or behavior to the unit manager/supervisor.
Supervises and assists CNA’s and Unit Helpers in all care areas assuring use of proper procedure to ensure that residents are receiving adequate assistance with personal hygiene and daily activities.
Gives report to CNA’s - giving them instructions on special concerns (i.e., V.S., I&O's, tray orders, etc.).
Receives reports from CNA’s at the end of the shift and as concerns are addressed.
Completes appropriate section of 24 Hour Shift Report by writing legibly pertinent data only.
Consults with unit manager or shift supervisor regarding the need for disciplinary action of employees; participates in disciplinary action as necessary.
Gives feedback to unit manager or shift supervisor regarding the performance of employees.
Ensures that weights and vital signs are taken and documented as ordered (i.e., monthly, weekly, daily).
Verifies information on Consultation Sheet prior to completing the forms and sees that residents are ready for all scheduled appointments on time. Orders early meal trays as indicated.
Assists the Nurse Manager/Nursing Supervisor with all necessary paperwork and transportation arrangements for emergency care for resident. Checks to see that transfer form is complete and accurate.
Obtains specimens, as ordered.
One hour prior to end of shift checks for completion of documentation by CNA's in the CareAssist system.
Completes paperwork on a daily basis, in a timely manner. Enters required documentation into CareAssist system as needed.
Reports accidents/incidents or medication errors to Nurse Manager/Nursing Supervisor and completes appropriate forms.
Maintains CONFIDENTIALITY of all resident care information.
Participates in St. Luke's Quality Assurance Program.
Assists the CNA's with resident care as needed.
Other related duties that may have become necessary or as directed by the supervisor, department head, and/or administration.
QUALIFICATIONS:
Graduate of an approved School of Nursing.
Current L.P.N. licensed in the State of New York.
Previous experience in geriatrics helpful, but not necessary.
Understands the special needs of the Skilled Nursing residents.
Understanding of and genuine interest in gerontology.
A willingness to learn through in-service and continuing education programs.
Must be able to supervise and instruct others professionally.
Must be well-groomed, courteous, tactful, patient, kind, and pleasant when dealing with residents, visitors, and co-workers.
