This position is a Relief Full-Time role until November 16, 2026.

Shift Type & Times: Days, 07:00 to 18:10, 4 on 4 off extended days.

About The Role

The Long-term Care Coordinator (LTCC) practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM), within a patient- and family-centered care model.

The LTCC is responsible for coordinating daily operations and ensuring quality individual care within designated clinical areas. This role supports seamless movement of individuals through the healthcare system and access to appropriate resources. The LTCC supervises, serves as a role model, and provides clinical decision-making, assessments, and family education. They review and recommend improvements on policies, procedures, staffing levels, and quality initiatives. The LTCC liaises with the interdisciplinary team for resource management and staff educational needs.
