Job description

Reporting to the designated manager within Home and Community Care, the Long Term Care Case Manager, Nurse, works as part of an inter-professional healthcare team. The role involves assessing, planning, organizing, reviewing, and evaluating the care needs of clients and their caregivers/families requiring community-based services. The nurse fosters continuity of care and promotes partnerships by liaising with primary care physicians, long-term care homes, hospitals, and other involved organizations. The position may involve work in settings such as Long-Term Care, assisted living, acute care, and community environments.

Qualifications: Education, Training, And Experience
  • Registration with BC College of Nurses and Midwives as a practicing RN.
  • Three (3) years recent related experience in gerontology, including two years in community health; or an equivalent combination of education, training, and experience.
  • Valid BC Driver's license.

  • Ability to apply nursing process as per BC College of Nurses and Midwives Standards of Practice.
